  • Section 295(c) — observed slicer to have old food debris around and on the blade.

  • Section 243 — observed single service items to be stored directly on the floor.

  • Section 295(c) — reach in cooler is observed to have heavy black and brown build up on the racks inside the unit.

Pizza Hut of Vincennes

  • Section 243 — observed several single serve items to be stored directly on the floor.

  • Section 433 — observed mop to be improperly stored and not allowing for air drying.

  • Section 295(c) — reach in freezer is observed to be soiled with old food debris inside and outside of the unit.


Casey's General Store #2271

3 Critical Violations

  • Section 345 — hand washing sink is observed to be soiled with a dark brown build up in the basin and on the surrounding edges.

  • Section 324 — observed sink in kitchen to have a leaking pipe with a bucket underneath the pipe, full of black water. Management was advised to contact maintenance and provide a work order/repair.

  • Section 177 — observed food to be directly stored on the floor of the walk-in freezer.

6 Non-Critical Violations

  • Section 243 — observed single serve items to be stored directly on the floor of the storage room and on the floor of the kitchen.

  • Section 295(c) — walk in freezer in the kitchen is observed to be heavily soiled with food debris and litter/refuse.

  • Section 295(c) — pizza oven is observed to be heavily soiled with old food debris and have a thick heavy dust accumulation on the inside of the unit.

  • Section 410 — light above the pizza oven is observed to be non-functional.

  • Section 295(c) — reach in cooler used for pizza toppings is observed to be heavily soiled with old food debris.

  • Section 297 — ice chute on the soda fountain machine is observed to have a brown and pink build up inside the chute and around the outside of the chute.

  • Section 439 — observed toxic materials to be stored directly next to food items and directly next to a food preparation area, and not preventing cross contamination.

  • Section 346 — observed employee hand washing sink to not have any soap or a dispenser available.

  • Section 347 — observed employee hand washing sink to not have any hand drying provisions available.

  • Section 433 — observed mop to be improperly stored and not allowing for air drying to take place.

  • Section 298 — microwave in lobby, for customer use, is observed to be soiled with old food debris and brown build up inside.

  • Section 243 — observed single service items to be stored directly on the floor.

  • Section 426 — observed storage area to have unused equipment and litter, and not preventing potential harborage conditions.

  • Section 295(c) — cutting board on the prep cooler is observed to be soiled with black build up, scratches, and heavy scoring. Cutting board can no longer be effectively cleaned or sanitized due to the poor condition.

  • Section 431 —facility is observed to be in need of a deep cleaning. This includes all areas of the kitchen, lobby, floors, walls, shelving, and equipment.

Vincennes Save-A-Lot

5 Critical Violations

  • Section 415 — harborage conditions are present in the food storage area. Area is littered with boxes, shelving, and unused/broken equipment that is no longer being used in the establishment.

  • Section 334 — utility/mop sink is observed to have a hose laying in the basin and not preventing back siphonage or allowing for an air gap.

  • Section 415 — back walkway in storage area is observed to have harborage conditions. Area is found to have broken/unused equipment, boxes, plastic bags, and litter throughout the walkway.

  • Section 204 — multiple meat trays used for transporting fresh cut meat out to display cases are observed to be contaminated with old food debris and blood.

  • Section 191 — observed beef steaks and chuck roasts out for retail sale, to be held past their "sell by" date.

7 Non-Critical Violations

  • Section 430 — numerous ceiling tiles are stained throughout the establishment.

  • Section 295(c) —fan guards in produce cooler are observed to have heavy black accumulations and dust.

  • Section 433 — mops are observed to be improperly stored and not allowing for air drying to take place.

  • Section 295(c) —fan guards in the meat cooler are observed to have heavy black accumulations and dust.

  • Section 295(c) — shelving in the meat cutting room is observed to be soiled with old meat pieces, old food debris, and blood.

  • Section 243 — observed single service items to be stored directly on the floor in the butcher area.

  • Section 295(c) — dairy cooler is observed to be soiled with debris, spills, and white fuzzy build up.
