Establishments with Violations

 

Dollar General Store #1161- 1001 North 8th Street, Vincennes

  • 2 Non-Critical Violations

    • Section 414- Establishments roof is observed to be in disrepair. Observed multiple areas in establishment to be leaking and causing damage to food products. Leaks are found to not be contained.

    • Section 106- Establishment is observed to not be able to promote safety for public health and ensure that food products are not contaminated.

  • 1 Critical Violation

    • Section 109- Establishment is ordered to cease operations until the issue can be contained in a safe manor.


*The manager was advised to contact Knox County Health Department when the establishment has contained issues and work orders have been submitted before reopening.


Follow-up Inspection

  • 2 Non-Critical Violations

    • Section 414- Establishments roof is observed to still be in disrepair. Leaks are observed to be contained at this time and a work order has been submitted.

    • Section 402- Tile floor throughout the establishment is found to have numerous missing, broken, and cracked tiles.


*Establishment has been approved to reopen. A follow-up inspection has been scheduled.


Hucks #135-1515 Washington Ave., Vincennes

9 Violations

  • Observed employee drink to be uncovered in food preparation area.

  • Handwashing sink in kitchen is observed to be heavily soiled with old food debris and black build up.

  • Observed entire food preparation area to not be properly cleaned and sanitized. Sanitizer was found to have no time stamp and test at Oppm.

  • Observed numerous foods to have no type of date marking system or to be held past expiration date and available for human consumption.

  • Multiple foods and food ingredients that have been removed from original package are found to have no type of identification labels.

  • Observed food to be stored directly on the floor in walk in cooler.

  • Clean utensils and food storage containers are found to be soiled with old food debris and grease.

  • Utility/mop sink hose is found to be laying in basin and not preventing backsiphonage.

  • Kitchen area is observed to be in need of a deep clean. This includes: all equipment, freezers, coolers, walls, floors, sinks, shelving, and hood vents.
